Microsoft word survey template radio buttons

I have created a fillable form in Word with a variety of drop-down boxes, radio buttons and text boxes. It all seems to work fine on my own computer have Windows 10 but I have to send out the form to clients and quite a few people have said that they cannot fill in the radio buttons on the form, although the drop-down lists and text boxes seem to be okay.

I am assuming that the ability to complete the form is dependent upon how the client's computer is configured, but I don't know how to identify the problem or how to amend my form so that everyone can open it.
